“Never” happened to California patients 1,538 times

Los Angeles Times: The California Department of Public Health reports there were 1,538 so-called “never events” at California hospitals last fiscal year. (“Never events” are considered completely avoidable and thus should never happen.) Molly Hennessy-Fiske describes some of those errors, including the case where a man with a tumor on his kidney had his healthy kidney removed in error by a doctor who did not have kidney surgery privileges at the hospital. The diseased kidney still had to be removed, leaving the patient on dialysis.
